Given that Value Network’s Partners care for medically complex and high-risk clients, Partners indicated the need for a tool to help with data collection and cross-sector collaboration. VN partnered with findhelp to create the Value Network Community Connector (VNCC), a social care platform to make and track referrals while gathering and extracting data.
What is the VNCC?
The VNCC is a technological system which breaks down traditional barriers between clinical and community care agencies. By allowing healthcare and social service agencies to work collaboratively within a technological system, social interventions are connected to client outcomes and performance goals.
VN Partners are using the VNCC to:
- Increase communication between community-based organizations, behavioral health entities, hospitals, and primary care providers
- Track closed-loop referrals
- Identify SDOH factors to inform prevention and early intervention strategies
- Help improve client outcomes to demonstrate their value
- Improve the overall health of the community
What are Social Determinants of Health?
Social Determinants of Health (SDOH) are “conditions in the environments where people are born, live, learn, work, play, worship, and age that affect a wide range of health, functioning, and quality-of-life outcomes and risks.” SDOH factors such as education, job status, family social support, income, and community safety can account for 50% of an individual’s overall health.
