Innovative Approaches to Better Health Outcomes
Value Network is a robust BHCC IPA that enhances quality care through clinical integration, the use of community-based recovery support services, and the collection and analysis of quality and service data to improve behavioral and physical health outcomes. By bringing Partners together to work in alignment on strategic approaches to service delivery, improved health outcomes and cost savings result.
In practice, our Partners work collaboratively to improve communication between all those involved in patient care, develop less fragmented care plans and better care transitions; address social determinants of health; reduce clinical and administrative redundancies; share best clinical practices; collect and monitor data and analyze performance against key health metrics; and reduce barriers to participating in value-based purchasing arrangements, including an improved and integrated model of care and the ability to understand and manage cost and quality.
As behavioral healthcare providers, our Network Partners have data-backed insight into the totality of patient health that primary providers alone do not. By leveraging that insight and focusing on shared outcomes and approaches, Value Network mitigates the high cost of behavioral health and addictions on overall cost of care.
