How to Access & Join the Trusted Network
The Value Network Community Connector is a web-based application and can be accessed on any device.
Anyone in the community can access the Community Connector as a resource directory by clicking here. However, Value Network Partners, Affiliate Partners, and Community Collaborators have access to additional tool functionality, premium data dashboards, ongoing training and support, and more.
All Value Network Partners have use of the tool, which include community-based organizations, primary care offices/IPAs, and hospital partners located within the 8 WNY counties. By providing this tool to Partners at no additional cost, Value Network is committed to improving connectivity and communication across the system of care in WNY.
